csvde, dsquery, admodify, joeware.net tools, or script would do that for you  
  
pretty easily.  I think there's a kb article that walks you through step by  
  
step as well.

Script would come to mind then since you want it on a web page. (unless you  
  
planned to print it to paper and scan it :)  
  
What language do you prefer for your web scripts?  If IIS, I suspsect

If you search on the web, you will find a free web-based address-book app  
  
that you simply unzip on an internal IIS server and populate the name of the  
  
domain in the global.asa file, IIRC. Worked like a charm once we got the  
  
permissions issues sorted out w/IIS authentication and ldap access.
